Student Plan

The insurance waiver/enrollment period for the Fall 2013 semester will begin on June 3rd and ends on August 30, 2013 at 5pm. 

Students do not need a referral from USF Student Health Clinic clinic to see a Doctor in Aetna's Preferred Network. Students can find an Aetna Doctor by searching DocFind.

Fall 2012: $729 (8/1/2012-12/31/2012) 

Spring/summer 2013: $1045 (1/1/2013-0731/2013)

Summer 2013: $421 (5/1/2013 - 7/31/2013)

 Academic Year 2013-2014

Fall 2013: $895 (8/1/2013-12/31/2013)

Spring/Summer 2014: $1241 (1/1/2014-07/31/2014)

Summer 2014: $538 (5/1/2014-7/31/2014) 

Note: Students who purchase Spring coverage are also covered for the Summer term.

Claims

To file a claim with Aetna, download the Aetna Health Insurance Claim Form, attach appropriate documentation, and mail to:

To find out the status of your claim, create an Aetna member account on the Aetna Website or call (877) 850-6036.

If you need help filling out the claim form, visit Health Promotion Services in UC 5th Floor.

Please note: Claims must be submitted to Aetna within 90 days from the date of treatment.

Finding a Doctor

Follow these instructions to find a doctor within the Aetna network.

  1. Go to Aetna's Doc Find website.
  2. Click on the "Enter DocFind" button.
  3. Click on the "Continue to DocFind" button.
  4. Enter the criteria to find a doctor near you.

Remember that you should always go a doctor within the Aetna network in order to keep your costs to a minimum. If you go to a doctor outside of the network, you will be responsible for paying a larger percentage of the cost. For more information on your health insurance plan, read the section on Benefits.
