Health and Immunization Requirements

The University of San Francisco has specific requirements regarding health insurance and immunization for students.

Health Insurance

As a condition of enrollment, the University of San Francisco requires certain groups of students to have health insurance, both to protect against unexpected high medical costs and provide access to quality care.

  • Off-Campus Students:

    • Domestic graduate students registered for 6 credit hours or more (excluding visiting students, certificate programs, and online programs) are automatically enrolled and charged for the health insurance.
    • Students who have comparable health coverage can opt out of this requirement each academic year.
    • Graduate students registered for 1 to 5 credit hours are NOT automatically charged but are eligible to purchase USF-sponsored Student Health Plan.
  • On-Campus Students:

    Graduate students residing in University-operated housing must purchase the USF-sponsored health insurance or apply for a waiver.
  • International Students:

    All international students registered for at least 1 credit hour or more will be AUTOMATICALLY enrolled in and charged for the USF-Sponsored Student Health Insurance Plan.


Download the USF Immunization Form and have it completed by a licensed medical professional. Mail, email, or fax the form to the Health Promotion Services.

  • Off-Campus Students:

    Graduate domestic students living off campus are NOT required to submit evidence of MMR (Measles, Mumps, Rubella) immunization.
  • On-Campus Students:

    Graduate students born on or after January 1, 1957, who are living in USF-operated housing must submit evidence of two MMR (Measles, Mumps, Rubella) vaccines.
  • International Students:

    All International students must submit:

    1. Evidence of two MMR (Measles, Mumps, Rubella) vaccines.
    2. Proof of Tuberculosis test administered within the last 12 months.

    3. Students with a positive TB test are required to submit a proof of chest x-ray administered within the last 12 months.

Submission Deadline for Immunization Form:

  • September 1 for students admitted to Fall and Summer terms.
  • February 1 for students admitted to the Spring term.

LATE FEE: If you do not submit your immunization form by the deadline, you will be charged a $100 late fee.
